GTCA Application Form and Instructions

For the saints who are led by the Lord to migrate to Madison or to serve full-time as part of the team in the Madsion area, please follow the instructions below:

Instructions for Submitting Your Form Electronically (recommended):

Download Form: Fall 2009 Field Team Application_Electronic Submission.doc

1. Download the Word document Application Form above and save it to your computer.

2. Fill out the first page of the application form and save the document.

3. Send the file to the leading or responsible brothers in your locality to fill out the second page of the form (recommendation form).

4. Have the leading or responsible brothers email the completed application form document to thechurchinmadison@gmail.com

 

Alternate Instructions for Submitting Your Form by email (no fax available):

1. Download the PDF document Application Form above to your computer and print the document.

2. Fill out the first page of the application form.

3. Have the leading or responsible brothers fill out and sign the second page of the form (recommendation form).

4. The form should then be scanned and sent via email to:  thechurchinmadison@gmail.com

 

When we receive an application form, we will send an email both to you and the leading brothers in your locality to confirm the receipt of your application. We will then have further fellowship as necessary.
